Athletic Department Forms
All students participating in athletics at Covenant must complete the required FHSAA paperwork. This paperwork must be completed and turned in BEFORE the first day of practice for the sport that the students wishes to participate in.
NO STUDENT WILL BE PERMITTED TO PARTICIPATE IN ANY PRACTICE
OR TRY-OUT WITHOUT THE PROPER FHSAA PAPERWORK.
All students must complete FHSAA Form EL2 (Preparticipation Physical Evaluation) and FHSAA Form EL3 (Consent and Release from Liability Certificate). These forms are revised by the FHSAA, so please click HERE to access the latest forms on the FHSAA website.
